Citizens of Ukraine aged 16 and over may independently change your name<\/strong>, surname or patronymic. This was reported by the Lviv Administrative Services Centre. To make a change, you need to submit an application and documents.<\/p>\n

Name change procedure<\/h2>\n

To change your name, you need to contact any territorial unit ASC<\/strong> at the place of residence. You must submit an application and a set of documents.<\/p>\n

The service is subject to a fee. The application is considered within three months from the date of submission, but this period may be extended for another three months.<\/p>\n

What documents are required<\/h2>\n

To change the name, you must provide:
\n- an application for a change of name;
\n- a passport of a citizen of Ukraine;
\n- documents confirming the change (if necessary);
\n- a receipt of payment of the administrative fee.<\/p>\n

You can apply to any ASC regardless of your place of registration. The service is available to all citizens who have reached the age of 16.<\/p>\n

Previously, only adults could change their name on their own. The innovation simplifies the procedure for young people.<\/p>\n
